• A summary of your achievements and activities demonstrating your involvement: provide clear, concise details about your volunteer work, school or community involvement, extracurricular activities, and artistic or athletic interests (500 words max)
• One (1) Community Reference Letter

Instructions for Community Referee

In your reference letter, please include:

1. Applicant’s full name and SFU student number (you can ask the student for this information)
2. Referee Information: your full name, email address, phone number, organization name and relationship to the applicant
3. Please describe the following:
   • The applicant’s contribution to their school and/or community;
   • The applicant’s leadership qualities and potential; and
   • Any other additional comments in support of the applicant.
