Special criteria also apply:

• At the time of submitting the application file:
  • Have a program college performance rating (R rating) of 24 and above
  • Be a member of a recognized sports organization (be a high-performance student-athlete)

Application Documents

1. Complete the online form.
2. Attach a transcript of your college diploma (PDF format).
3. Attach a certificate of your sporting involvement signed by a person who trains you, teaches you or supervises you
